During the surveys, please make sure the 'Continue' button at the bottom of the page is pressed (you may have to scroll down). This saves the information to our database and sends you to the next page.

Once the forms have been completed, your will be sent a confirmation email.

Sign up for Your New Health Screening

To sign up for your new Health Screening, enter the company code given to you by our office or your employer and select either Flu Shot Only or Healthscan (which has the option to add a flu shot). You will be directed to a page that will collect the answers required for your health screening.

Before you begin, please make sure you have the following information available:

Patient Name, Address, Phone Number

Date of Birth

Insurance Card with Insurance Name and Policy Number

Company Code

Screening Type
Flu Shot Only (No Additional Bloodwork)
Healthscan (Full Health Report)
